This is an ongoing project to support the development of shared, research-supported, standardized practices in our field while encouraging creative freedom in clinical approach and intervention design.
The below template was developed as a one-size-fits-all attempt to streamline planning and documentation processes between populations, institutions and job types onto a single page! Quite a challenge to get it all to fit. I have use this template (or variations of it) with the following populations; adolescents w/ behavioral emotional needs, geriatrics (groups & individuals), dementia/parkinsons, multigenerational & community music therapy, acute adult psychiatry and medical music therapy. It has been used for both private practice and clinical practice.

I am also in the process of creating templates for invoicing, progress notes and intake documentation with a long-term goal of making these materials accessible to the MTBC community.
